Then print two copies, one to send with your check to Dancin' Z Productions, and one for your own records.

Read The Fine Print!

If anyone has any trouble downloading our registration forms, please e-mail me at Greg Michelle and I will email the forms to you.

  1. All UCWDC Competitors must pre-register by May 15th for both the Event and the Contest you'll be competing in.
  2. Deadlines exist for special pricing. Do not delay mailing your forms in. All advanced registration fees must be paid prior to the deadline to get special rates.
  3. Advanced registration fees are Not Refundable for any reason unless we receive your request prior to May 10.
  4. Personal / Company Check and / or Credit / Debit / ATM Cards will not be accepted on site at the event. Cash, Travelers Checks and Money Orders Only. Do not mail Cash. Credit / Debit / ATM Cards are not accepted at anytime.

Online (or e-mail) registration is not available.
